External and internal snapping hip

Snapping hip can be external/ extra-articular (trochanteric) or internal/ intra-articular (inguinal).

It is a pathology of friction or abnormal contact between a tendon structure and a bony prominence. In the external case, the friction occurs between the ilio-tibial band, gluteus maximus and tensor fascia muscles and the greater trochanter femoris. In the internal case, friction occurs between the psoas-iliac muscle and the ilio-pectineal eminence or transition between the femoral neck and head.

As queixas passam por estalido ou click dentro da articulação ou à sua volta e pode estar associado ou não a dor concomitante. Provoca a sensação que algo se move repentinamente dentro ou fora da articulação durante determinados movimentos. O click pode ser audível pelo próprio doente e pelo médico. Frequentemente os doentes referem que algo saiu do lugar. Quando há dúvida diagnóstica e após realização dos exames de estudo articular poderá ser necessário recorrer a uma infiltração guiada através de ecografia para realização de teste diagnóstico anestesiando o tecido doloroso.

Treatment of the most frequent mechanical causes involves physiotherapy, using physical agents, massage, muscle re-education and myofascial release techniques. It can be useful to use analgesics and anti-inflammatory drugs during the most exacerbated period of pain. Local infiltration can also be used to control the pain when it doesn’t respond to treatment.

In cases which are ultra-resistant to all conservative treatment, it may be necessary to surgically release the chronically inflamed tendon, but always after confirming the diagnosis and finding another cause of joint click. This liberation can be done in most cases by arthroscopy.
